Futures Planning Roadmap (7 steps)

Step 1: Clarify the person’s vision

Outcome: A clear picture of what matters most.

Include preferences, strengths, interests, culture, identity, communication, and what a “good day” looks like.

Step 2: Map current supports

Outcome: A snapshot of who provides support and how.

Include unpaid supports (family/friends/community) and paid supports (providers, DSPs, programs).

Step 3: Identify risks and gaps

Outcome: A prioritized list of what needs attention.

Common areas: health, housing, staffing, transportation, safety, benefits, and social connection.

Step 4: Build the plan

Outcome: Clear roles, action steps, and timelines.

List who will do what, which services to pursue, which documents are needed, and target dates.

Step 5: Prepare for transitions

Outcome: Reduced disruption when change happens.

Plan for caregiver changes, housing moves, staffing turnover, and medical events.

Step 6: Put it in writing and share it

Outcome: A practical plan others can follow.

Create a Futures Plan, a 1-page profile, and an emergency plan. Share with trusted supporters and providers.

Step 7: Review and update regularly

Outcome: A plan that stays accurate and useful.

Review at least annually and after major changes (health events, moves, service changes, losses).
